Program Overview

The Community Health Outreach Program serves as a major community-engagement platform, bringing health education, preventive-health awareness and appropriate support activities directly into communities.

HS-PHI takes a community-centered, evidence-informed approach. Activities are designed around local needs, delivered with relevant stakeholders, and reviewed for learning and improvement.
